Kenyans urged to go for heart screening to reduce premature deaths

By DANNISH ODONGO, NAIROBI, Kenya, Sept 30 – The Kenya Cardiac Society (KCS) has implored Kenyans to improve their health awareness by going for heart screening to reduce premature deaths caused by cardiovascular disease which is the world’s number one killer.

Speaking during the World Heart Day Friday, Dr Bernard Gitura – cardiologist and president of KCS, highlighted that heart conditions are on the rise in the country due to low public awareness on heart health specifically on the conditions that are contributing to the accelerated rise in cardiovascular complications.

Dr Gitura added that for those already on treatment for conditions such as hypertension, awareness needs to be created on how to comply with taking medication since most patients still don’t understand that treatment needs to be consistent.

In 2015, a nationwide survey on non-communicable diseases by the Ministry of Health found that 23.8 percent of Kenyans had raised blood pressure, yet 56 percent had never been screened for it and 39 percent of the population is overweight or obese.

Ashling Mulvaney, Senior Director for Healthy Heart Africa (HHA) outlined the importance of partnerships in tackling the burden of cardiovascular disease in Kenya.

“If we are to tackle cardiovascular disease in Kenya, stakeholders across public and private sectors need to work together to ensure that heart health awareness is raised, treatment is available and training is provided,” he said.

“We are working with KCS and the healthcare community to mark World Heart Day because we believe in the importance of partnerships in tackling the burden of cardiovascular disease in Kenya through awareness, screening and health sector strengthening.”

Cardiovascular disease including heart disease and stroke claims 17.5 million lives each year making it the world’s leading cause of death.

'Mood spreads from person to person via social contagion'

A new study in young adults has suggested that moods are contagious, and that teenagers are susceptible to "catching" their friends' emotional states.

Researchers suggest that prevalent moods in groups of adolescent friends are 'contagious.
According to the latest data available to the National Institute of Mental Health, approximately 3 million adolescents aged between 12 and 17 in the United States had at least one major depressive event between 2014 and 2015.
But although cases of diagnosed depression are reaching worrying numbers worldwide - the World Health Organization (WHO) estimate that around 300 million people are affected worldwide - many people, and especially adolescents, exhibit depressive symptoms that are just under the threshold for a clinical depression diagnosis.
This is called "sub-threshold depression," and the fact that it is not a clinical condition means that many people do not get the support that they need, despite often reporting a poor quality of life.
Now, researchers from the University of Warwick in Coventry, United Kingdom, are looking at how adolescents' social circles can influence their moods, in an effort to better understand what determines depressive symptoms among teenagers and what might alleviate them.
Robert Eyre, a doctoral student at the Complexity Science Doctoral Training Center at the University of Warwick, led this study.
"We investigated whether there is evidence for the individual components of mood (such as appetite, tiredness, and sleep) spreading through U.S. adolescent friendship networks," Eyre explains, "while adjusting for confounding [factors] by modeling the transition probabilities of changing mood state over time."

Their findings were recently published in the journal Royal Society Open Science.

Prevalent moods influence social contagion

Eyre and team note that existing studies have already started mapping a concept called "social contagion," suggesting that our social environments impact our emotional profiles.
Some research more specifically targeted teenagers, showing that adolescents with strong friendship groups tend to have better mental health.
"Previous studies have found social support and befriending to be beneficial to mood disorders in adolescents," Eyre points out, "while recent experiments suggest that an individual's emotional state can be affected by exposure to the emotional expressions of social contacts."
And now, the research team has looked at whether or not a teenager's friendship group can influence various mood changes, and how drastic these changes can be.
"Clearly, a greater understanding of how changes in the mood of adolescents are affected by the mood of their friends would be beneficial in informing interventions tackling adolescent depression," says Eyre.
The researchers analyzed data collected by the National Longitudinal Study of Adolescent to Adult Health, which worked with U.S. adolescents in grades 7 to 12 in the 1994 to 1995 school year. The participants were followed until 2008, when individuals in the cohort had reached adulthood.
By looking at the data on the U.S. adolescents' friendship networks and mood changes, the researchers found that the prevalent moods in a circle of friends can influence the emotional state of the individual adolescent.
Eyre and colleagues noted that depressive moods are, in fact, "contagious." Symptoms of this include feelings of helplessness, tiredness, loss of interest, poor concentration, sadness, and feelings of worthlessness.
However, even though low moods and feelings of sadness could be spread among friends, they did not typically push individual adolescents over the threshold into clinically diagnosable depression.
Mathematical models showed that the more friends exhibiting low moods in an adolescent's social circle, the more likely it was for that particular teenager to experience similar symptoms.
At the same time, the study showed that a circle of friends in which most individuals are upbeat and optimistic can improve a teenager's mood and relieve their symptoms of depression.

Tackling 'spreading of negative mood'

The researchers suggest the necessity of a twofold approach for improving teenagers' low moods. As well as encouraging them to develop friendships, efforts should be made to stop negative emotions from perpetuating in adolescents' social circles.
"Understanding that these components of mood can spread socially," they write, "suggests that while the primary target of social interventions should be to increase friendship because of its benefits in reducing of the risk of depression, a secondary aim could be to reduce spreading of negative mood."
Co-study author Prof. Frances Griffiths, from the University of Warwick Medical School, explains that the research could be harnessed to improve current approaches in health policy, especially targeting sub-threshold depression in adolescents.
She says, "The results found here can inform public health policy and the design of interventions against depression in adolescents."
"Sub-threshold levels of depressive symptoms in adolescents is an issue of great current concern as they have been found to be very common, to cause a reduced quality of life, and to lead to greater risk of depression later on in life than having no symptoms at all," concludes Prof. Griffiths.

Linking depression, insomnia, and the brain's reward center

Sleep disturbance is a significant risk factor for depression.
Breaking research finds that increased activity in a brain region responsible for reward-related functions reduces the risk of depression related to sleep problems.
Major depression is responsible for almost 4 percent of the disease burden in the United States. In 2015, around 16 million U.S. adults aged 18 or older reported having had at least one major depressive episode in the previous year.
Although there is a range of treatment options for people with depression, there is no cure and we still have much to learn. Because of this, research into the brain activity associated with depression is paramount.
The precise causes of depression are not always clear, but certain factors are known to play a part: poor sleep, for instance, is a relatively common risk factor.

Sleep disturbance and depression

Although insomnia and hypersomnia, or excessive sleepines, are both symptoms of depression, insomnia is more strongly associated with the severity, onset, and recurrence of depressive episodes.
In fact, people without depression but with insomnia have twice the risk of developing depression when compared with those who sleep well. Along similar lines, research has also shown that in certain individuals, depressive symptoms improve if sleep issues are relieved.
Over recent years, researchers studying depression have focused more and more attention on individual differences in brain function.
In particular, a region called the ventral striatum has yielded interesting findings. This is an area of the brain involved in reward, motivation, and goal-directed behavior.
Experiments have demonstrated that reduced reward-related ventral striatum activity is associated with depression. Also, deep brain stimulation of the ventral striatum has been shown to have an antidepressant effect in people with treatment-resistant depression.
It seems that high levels of reward-related ventral striatum activity may buffer the individual against the effects of negative experiences, reducing the likelihood of developing depressive symptoms.

Ventral striatum and depression investigated

A study published this week in the Journal of Neuroscience took a look at these theories in more depth and examined whether or not reward-related activity in the ventral striatum influenced the relationship between sleep disturbance and depressive symptoms.
The research was headed up by Reut Avinun, Ph.D. - from Duke University in North Carolina - who recently spoke with Medical News Today. She explained that this research follows on from two other studies at their laboratory demonstrating how the same brain region could modulate the effect of stress on depression.
"[The studies] showed that individuals who experienced stress and had high reward-related activation in the ventral striatum were less likely to report on depressive symptoms."
For their latest investigation, they enrolled 1,129 young adults drawn from the Duke Neurogenetics Study. Firstly, the participants completed a questionnaire about the quality of their sleep. In total, 35 percent of participants were characterized as "poor sleepers."
Next, they played a card-guessing game wherein they received positive and negative feedback designed to trigger the ventral striatum. As they played, the researchers collected functional MRIdata.
They found that individuals with higher reward-related ventral striatum activity were significantly less likely to report symptoms of depression when experiencing poor sleep quality. This result remained significant even after controlling for factors such as age, sex, race, early or recent life stress, and symptoms of anxiety.
Dr. Avinun summarized the findings for MNT.
In our study, we showed how the negative effects of poor sleep can also be modulated by [the ventral striatum], so that high reward-related activation can buffer the effect of poor sleep on depressive symptoms.
